For a market as large as Houston, there are only 579 Neilson boxes?
With all the technology out there it really is insane how TV ratings are determined.

Not surprising. Most of us have no access to watch.
In general, the overnight ratings are based on the boxes, but the diary users which are many more report sometimes a week later since they require some degree of hand tabulation.
This is why network broadcasts tend to have worse overnight (relatively) than cable because many people that watch antenna are on diary.
