This is a new construction home in the Houston area and our3rd party inspector found this crack in the brick. 4' crack from a window on the 2nd story to a window on the first. How bad is this? It's through the brick, not just the mortar like typical settling. Much of this wall was replaced during construction due to not meeting their quality standards. Just trying to get a handle on what we should be looking for as a fix as well as understanding what caused it in the first place. No other signs of foundation movement inside or out and the slab was poured about 7 months ago. Thoughts are appreciated!

Looks like the lentil settled, perhaps was not anchored well to the header

They should not be attached to the header!

I wonder of the header did not extend far enough past the opening.

Depends on the size of the opening and how much masonry load is above.

OP

How wide is the opening on the lower window ?

The picture almost looks like it is a garage span below

I've always understood that if it's a wood framed house, you did not want to attach your headers to the wood due to the movement of the wood.

These are the best pictures I have of the framing. The first floor has 3 windows each around 3' wide. 2nd picture is from the view inside of the house.

Are the lower headers 2x6 ?

Is this in a city with inspections ?

Yes on them being 2x6's and the city of Houston has done inspections along the way.

Can't speak for Houston but every 2 story house I ever framed back in the day in BCS was done with 2x12 headers on the first floor. In the triple windows below, are there 3 separate headers or one bigger one ?
