as wild as this is spreading in cities, you would think there would be a lot more people with mild symptoms or worse.

1% of the population is already dead/critical , possible that after the outbreak extreme social distancing went into effect and a large chunk of the population did not get exposed

The number of infected is still a very, very small percentage of total population of these cities.

If you scale those numbers up for the population of NYC you end up with tens of thousands dead.
