Pretty positive article with lots of caveats on the larger treatment implications.

https://www.statnews.com/2020/04/16/early-peek-at-data-on-gilead-coronavirus-drug-suggests-patients-are-responding-to-treatment/

Quote:

A Chicago hospital treating severe Covid-19 patients with Gilead Sciences' antiviral medicine remdesivir in a closely watched clinical trial is seeing rapid recoveries in fever and respiratory symptoms, with nearly all patients discharged in less than a week, STAT has learned.

Remdesivir was one of the first medicines identified as having the potential to impact SARS-CoV-2, the novel coronavirus that causes Covid-19, in lab tests. The entire world has been waiting for results from Gilead's clinical trials, and positive results would likely lead to fast approvals by the Food and Drug Administration and other regulatory agencies. If safe and effective, it could become the first approved treatment against the disease.

The University of Chicago Medicine recruited 125 people with Covid-19 into Gilead's two Phase 3 clinical trials. Of those people, 113 had severe disease. All the patients have been treated with daily infusions of remdesivir.

.........

"It's always hard," she said, because the severe trial doesn't include a placebo group for comparison. "But certainly when we start [the] drug, we see fever curves falling," she said. "Fever is now not a requirement for people to go on trial, we do see when patients do come in with high fevers, they do [reduce] quite quickly. We have seen people come off ventilators a day after starting therapy. So, in that realm, overall our patients have done very well."

She added: "Most of our patients are severe and most of them are leaving at six days, so that tells us duration of therapy doesn't have to be 10 days. We have very few that went out to 10 days, maybe three," she said.

Just like the HCQ studies, would really like to see this anti-viral given on symptom onset to avoid hospitalization in the first place.

Results so far very encouraging

First, it's great that we are finding existing drugs to combat an unknown virus.

But I find it hilarious that this article is being praised, with 2 deaths out of 125 test cases. The same death rate as no treatment. Which is the argument the death dealers have been using against HCQ. Difference being Trump's cheerleading of course.

2 deaths out of 125 SEVERE HOSPITALIZED PATIENTS would be far below the expected death rate. You're looking at OVERALL rate, which includes asymptomatic, mild, or mild/moderate cases.

Not the same. With only a small number of exceptions, these were severe cases, which have a much higher mortality than merely identified cases.

That is also well below the average length of hospitalization.

It is sort of concerning that this isn't some official release of the data and is coming from a website that I'm not familiar with. I hope this is all true.

They basically intercepted an internal communication from the University of Chicago staff and ran with it. Since the trials are all ongoing, everyone has said "no comment"

problem with Remdesivir is it's given by IV

so it's not a good prophylactic at this point

I did my Holiday Inn Express google search and there is literature on Remdesivir showing Prophylacic potential during MERS experiments. Hopefully there is some potential here as well.

Statnews is legit, oriented for pharm industry.

Edit to add: How are Gilead's futures looking?

Yea Statnews is pretty legit with good reporters

Unless they are talking **** about the biotech you are invested in, then its fake news and they are shorting the stock to make a profit by releasing false info

Jim Cramer was raving about this like 5 hours ago.

Here is a macaque study on remdesivir given 12 hours post inoculation with virus

Appears it reduces viral load in lungs but not upper respiratory track...which would help you if you got infected, but not from shedding and infecting others

Great..... now we just need to figure out how in the world they can boost their production and get it to the field.

What's the oral bioavailability of Remdesivir? Edit found it F=essentially zero

So the theory is they "create" antibodies" to Covid and give them to patients? Sorry, I'm a rube

Quote:

Remdesivir is not suitable for oral delivery as its poor hepatic stability would likely result in almost complete first-pass clearance.

Additional it was investigated that the IM route was also suboptimal for the administration of remdesivir. Studies assessing IM administration did shown a slow and variable release from muscle, evidence for IM metabolism, and delayed appearance of the pharmacologically active triphosphate GS443902 in PBMCs. Therefore, it was concluded that IV administration delivers GS-443902 more rapidly and consistently into target cells.

no, you're right on

they identify a couple of antibodies they see as being most effective, produce them as a cocktail, and ship them out as injections

they're effectively prophylactic booster shots but they can also be used after catching the bug as well
