Another very interesting video from Medcram today. He spends a great deal of time talking about the pathophysiology of the effect of the virus' blockade of ACE2 receptors in blood vessels and how this is leading to inflammation via significant oxidative stress.

Later in the video he discusses the hospitalization data coming out of New York that seems to indicate people with asthma and COPD did NOT seem to have an elevated risk of hospitalization. However, people previously diagnosed with cardiovascular disease (including hypertension) and obesity were much more likely to need hospitalization.

This is another potential reason why some older people are more affected and some (kids and young people) aren't as much.

This was one of the early mysteries of this disease, if it was a lung disease as we originally thought then why are diabetics, HTN, heart disease and obese patients dying so much more than COPD, asthma and other lung diseases. Now we believe that it is not a true lung disease but an endothelial disease and it is all starting to make sense.

Now that we have a better understanding on the mechanisms of this illness I think the treatments and outcomes will begin to improve.

My son had an appointment with his allergist last week and I asked him if we should take extra precautions with his asthma. The allergist said everything he was reading indicated that you aren't at a greater risk of a poor outcome with asthma.

Doc, don't want to make this too political, but it seems to me we should have known this in early February. Why was this kind of info not available in the early part of the year given how long China has grappled with this disease?

Likely because getting any kind of factual and useful information out of China has been near impossible.

There's no doubt about that, but shouldn't the clinical info have been out there earlier to lead everyone to this conclusion?

Have y'all seen a significant difference from uncontrolled hypertension versus hypertension under control with medication?

Yes controlled HTN is doing better but it is still considered a risk factor! Read my updates from Wednesday and Thursday on my Facebook page. I go into the mechanism of the angiotensin cascade. And you will see how ARB's and ACE inhibitors could potentially even help treat the disease. Be sure to read this one and the next day as I simplify it the best day.
