A couple days ago we noticed that my 4 year old had weird red spots on top of the toes, which we soon learned is a symptom and known as Covid Toes. There was no pain associated with the redness and it went away after about 36-48hrs
We called our pediatrician and were basically told that we can't get a test without the presence of a fever. This is in East Texas. We are a family of 5, have done a good job staying at home/away from others, and only going out to grocery stores about once a week. Not strict lockdown, but have done a good job following instructions. No one in the family has shown any symptoms of Covid. Not sure if it is a coincidence or if we should start trying to find another way to get tested.

I'd keep checking about the testing. They are opening up free testing in many areas of the state, no symptoms required. (I assume it is to provide a baseline of how many people show signs of the antibodies....)

If this is coming from your doctor, try the local health department, a different doctor, or call the local hospital. I'm pretty sure there are places and ways almost anyone can get tested with a little effort.

COVID toes?

Yes, it's an actual sign for some kids/teens. Unsure of why it manifests this way. Most kids are otherwise asymptomatic.

I am guessing we could call around and maybe find somewhere, but most places are directing us to call an automated hotline through the local hospital.
There is one outpatient place that can test using swabs, but I don't know if they can do antibody testing on me.

Of course toddlers get rashes, spots and all kind of things. It's not necessarily related to Covid-19. I would not quarantine for it until confirmed. If it went away, I just wouldn't worry about it personally.

Why don't you let fire ants bite your toes, and tell us if there is any pain associated with it?

Yeah sounds like it could even just be hand, foot, and mouth.

We had a family member who is in dermatology look at lots of pictures. Doesnt look like ant bites or hand, foot and mouth. But it is also odd and generic enough to be just about anything, but covid is on the long list of possibilities. We aren't super concerned. Like many on here, I would prefer it be covid, bc it likely means we have the antibodies.

I'd guess confirming it's not something else.

Letting the rest of the family know they've been exposed, so they can watch for their own symptoms, and stay away from others so as not spread the virus further.

Really seems odd that two months into this you don't easily connect the dots that if it's in your household, you should NOT be going out for any reason and self quarantining for the sake of your friends and community. Getting everything reopened depends heavily on people acting responsibly.

Here is a picture of what the toes looked like. It is crazy that a day later the toes were back to normal

Doesn't covid toes present more as consolidated papules and less as a spread out rash? From the pictures I've seen, it looked more like big red blisters
Willie Lane

There isn't a lot of info on how exactly it presents. I am reading that many people describe it as frost bite, which doesn't match up with my pictures.

That definitely does not look like HFM.

That actually does look like Covid toes!

This is potentially caused from increased clots being formed and causing vascular problems in the toes. This is a form of vasculitis which we are seeing with Covid-19. Since this is a vasculitis and likely a benign condition it could be a sign of a more risky problem like Kawasaki's we are seeing as this is also a vasculitis caused by your own immune system. We don't know all the details yet of Kawasaki's/ But we are usually not seeing Kawasaki's until 3-4 weeks after Covid-19 infections. But, I think I would talk to your doctor about considering using low dose aspirin to decrease the risk of Kawasaki's Disease. The guidelines for Kawasaki's is a dose of ASA to 3 to 5 mg/kg once daily for 6 to 8 weeks. That might be reasonable to do now.

Also 60-100mg of CoQ10 to help stabilize any endothelial damage for 3-4 weeks might not be a bad idea. We know that while very rare we are seeing the Kawasaki complications about 3 weeks after the initial Covid-19 infection so anything you can possibly do to prevent this is probably worthwhile. These treatments have very low risks of adverse reactions but possibly could prevent something bad in the future.

I would not suggest aspirin for a child unless the diagnosis is proven. Reye's syndrome can be an end result of aspirin given to a child.

Update: She has >101 fever today. Does that make sense? I thought covid toes were after infection.

Have you contacted her pediatrician? I would definitely be calling...right now and talking to the on-call doctor.

Yes. Testing scheduled for 8:30 in the morning. Our pediatrician also got the local pediatric cardiologist in the loop just in case.

Sounds good calling pediatrician and very responsible of him/her to call cardiologist. I would watch for symptoms of Kawasaki's I posted them the other day on my facebook post.

How is your daughter doing?

She is still running 101-102 fever several times a day. Motrin is helping to bring it down. Tylenol doesn't do much for her. She hasn't shown or complained about any other symptoms. Yesterday we started her on 5mg/kg of ASA and the CoQ10 arrives from amazon today.

They took blood for labs yesterday, I know one of the panels is pro inflammatory cytokines. Drs are very aware of Kawaskai Disease and looking out for it.
Dr. called in a COVID test, but we still haven't gotten a call back from the testing facility. They ordered a swab test. Not sure if we should have or need to push for an antibody test.

Lab came back with elevated CRP (was 7). Everything else was normal.

Quick Update: Daughter ran fever a couple days and then it stopped. Her swab test came back negative for COVID.
