Other hospital docs - what is your availability of core treatments?

With the surge of admits we are getting scarce. For me:

Plasma: pretty fair. 1-2 day turn around.

Remdesivir: day to day. Tenuous. Takes ID approval which sometimes means 1-2 days because of ID docs cover multiple hospitals.

Tociluzimab: Ran out today. Takes pulmonary approval. So many stormers I have been pulling the trigger like a M-16 lately. I have a 48 y/o man teetering the drain. Luckily pharmacy borrowing 400mg dose from another hospital.

Dexamethasone: fine

Anticoagulation: fine.

Not a doc, but we are at about 1-2 day back order shipping plasma and beating ourselves up. Glad we aren't the only ones that far behind...

Sure wish I could figure out how to get more donors across the finish line.

Honestly, I don't know if I would have known about the need for plasma if I hadn't been reading this forum since the beginning.

Is there any way to direct a public relations campaign toward doctors who treat Covid so that they can encourage recovered patients to donate?

On another note, your reminders to donate blood caused us to donate at Carter last week. So, keep it up! (Negative for antibodies or I would schedule my plasma donation next)

The messaging is starting to get out regarding plasma. Was in a patient's room last night and saw a commercial featuring Dwayne The Rock Johnson telling people to donate. First time I've ever seen anything in terms of a public campaign. Hopefully that will get things going.

Not a doc but:

In terms of treatment, we've never had Toci as far as I know.

We ran out of initial stock of Remdesivir but I think we've gotten access to more.

Plasma takes a day or two. Still part of the Mayo Clinic study so it apparently is helping with sourcing. Several doses apparently have come from from NY.

Haven't heard anything about shortages of dexamethasone or heparin. Heparin is the one that I was most concerned about considering it's randomly been on backorder in the past. Anecdotally, I think the fact that we are proactively anticoagulating most of these people is what has really helped our death rates go down.

We have imported probably 500-750 doses from New York blood center just to try and keep up with demand. They are drawing some huge numbers there.

Im giving whole blood tomorrow, but We Are Blood RR is apparently not doing serology tests. If I could get an antibody test, I would be more inclined to give plasma 3-4 times a month if i knew i'd be helping out.

Red Cross is doing it...you'd think the demand for convalescent plasma was so high they would be testing as a baseline to help identify more donors.

We need some uniquely American marketing campaign to incentivize folks.

Which anticoagulants? 81 or 325 aspirin? Coumadin? Heparin? Would Plavix work? Are any patients already on these when they are admitted? Co-morbids on the 48 year old?

Have you recovered from COVID? You may be eligible to give convalescent plasma through we are blood. I know they are drawing units as we have shared inventories with them.

If you really want an antibody test, drive up to Waco and give at Carter. ( I recommend giving in your local area though, as that's where it benefits the most)

ETA: just having the antibody is not enough to qualify for convalescent plasma, in order to qualify you also have to have been symptomatic. There is always another rule.

2nd edit: ARC is testing but they do not have the capabilities to draw the plasma. They actually send us their list of eligible donors locally and we are contacting them. They have a great marketing department and brand recognition but lack in operation capacity functionality. ( my center is the opposite, we are a lean operating machine that will go to any legal lengths to get product, we just don't have the $ to show for it. Lol)

Can I donate plasma if I have weird antibodies from pregnancy? I've always been told to just do whole blood, but I want to help if I can.

Donating blood tomorrow to see if I even have Covid antibodies, so this might be a moot question.

You'll need to be HLA rested. HLA antibodies cause TRALI (transfusion related acute lung injury). Usually the blood center will run the test for free when you give whole blood. Just tell your phlebotomist. Once you are tested, and negative you are eligible for plasma or platelet donations.
