Concerned my Dad May Have It

He had a slight fever a few days ago but didn't last longer than a few hours. Now he says he can't really smell or taste much today. He just got tested but won't get results for 1-3 days. He says he feels fine now, but when I talked to him he didn't sound like he was feeling fine. He's very stubborn about going to the dr and tends to ignore stuff like this. He's been needing knee replacements for years now and flat out refuses to do it. He's 68 with no real health issues other than his joints being worn out and he has asthma. Should we expect his symptoms to get worse, or is it likely this is bad as it gets since it's already been a few days since he had a fever? My mom was feeling bad yesterday too but said she feels much better today. They are both outside working in the yard/garden even though I've told them to take it easy and rest for a few days. I appreciate any thoughts or suggestions.

There's not much in the way of treatment for mild disease, however given his age and history of asthma he needs to be willing to pull the trigger quickly to seek medical care should he start having significant respiratory difficulties, fatigue, mental fog, etc. I know that's hard to convince some parents to do sometimes.

Watch him closely for another week to a week and a half. Watch for a plateau on recovery. Sometimes the body compensates as you get worse, so it just looks like you're staying the same. Eventually the body can't keep up and you require medicine to help.

Regarding his knee replacements, I'd recommend trying to figure out why he's so hesitant. My dad needed a shoulder replacement, and could've probably used knee replacements as well. When he got really sick, one of the limiting factors to his recovery was his shoulders. He tried to do physical therapy but just couldn't. And his activity level declined as his years went on because of his shoulders and knees. If your dad is putting off the knees, remind him that replacing them will allow him to regain more activity. That increased activity will ultimately allow him to retain more muscle, more independence, and a better quality of life than if he waited until much later, after he's lost the muscle due to inactivity from pain.

Slightly off topic, but I do feel where you're coming from.

Thanks. I'm two hours away from them but my brother lives next door so he should be able to keep an eye on them.
I don't know why he refuses to get his knees replaced. He most likely needs a shoulder replacement as well but he just flat out refuses to admit anything is wrong and that he's fine. I watch him nearly fall down multiple times walking from the barn to the house but he blows it off and ignores it. He's so damn hard headed, it's frustrating. He's only had one surgery in his life due to prostate cancer. I think part of him refusing to get any kind of operation done is he would finally have to admit he's getting older and can't do things the way he used to anymore. Although getting a knee replacement would allow him to keep going even more, but we can't get him to see it that way.

I think some older folks have a hesitancy for knee replacement because when they were younger, they knew someone in an older generation that had knee replacements and they didn't go well. I know a guy like that, he needs it bad, but his dad had it done 20-30 years ago and he had problems.

A lot of my family got together and shared covid around the thanksgiving table. One of my uncles tested positive the day after thanksgiving and today went to the er and was diagnosed with pneumonia. Keep an eye on your dad and make sure he's not overestimating his health and overcompensating until the body just can't keep it up.

I don't know if it'll be of any use, but if you need help convincing him I can give more details about my dad and answer questions he may have.

I'm not an orthopedist, but I do know a little about the surgeries and have seen many done in the OR. The technology is getting much better as well. It used to be common for them to wear out, or for them to get loose and need a revision. But odds of that happening are pretty slim these days.

Or, maybe just getting him to get a steroid shot or synvisc (artificial lubricant) shot in his knee will make him realize how much better things can be. If it works, he can get them every few months as needed.

The lubricant shots in the knee can help a lot. I have a family member who is not a good candidate for knee replacement surgery, but gets the lubricant shots every 3 months. The shots are the only reason she's be able to walk the last few years.

Well he has gotten worse. I ordered a o2 saturation meter and had it sent to them. This morning he was below 92 for an extended period of time and even dipped into the 80s some. My mom dropped him off at the ER and they did chest x-rays, but he's been sitting in the lobby for over four hours now. He's been told they have no rooms for him. Surely they can do something besides just letting him sit there? Should I be pressing them to do something?

Where are they? I've heard of this happening at numerous Dallas/Collin County hospitals lately. The good thing is that he's at the hospital if things get worse.
I know several people who spent a few days in an ER bed before getting a bed in the hospital.
I'm not trying to be discouraging, just letting you know that it might not be better at another hospital.
I would definitely stay in touch and encourage him to advocate for himself if he has difficulty breathing or begins to feel worse.

How low in the 80s did he go? And what was he doing when he got that low? If he was sitting down or laying in bed for a long time and then checked, what happens sometimes it the lungs slowly shrink when you're not active and taking deep breaths. If it happened when he was walking around and trying to be active, and he felt a little short of breath with minimal activity, that would be more concerning.

I trust they'll do the best they can. If they thought he needed care above someone else already in the ER they would've found a way to make room or told him to go to another hospital. That being said, what they saw in their initial triage assessment isn't going to be how he remains for the next 4+ hours. They should be reassessing him every hour or 2. And people should hopefully get discharged or move upstairs and make room for him.

ETA: Something he can do at home to ensure good lung recruitment is something called an incentive spirometer. He probably doesn't have one, but if he just takes in several breaths as deep as he can, and then slowly exhales against pursed lips for as long as possible, it'll help prevent something called atelectasis (the lung shrinkage). Do this for about 3-4 minutes every hour, along with little walk abouts. Additionally, tummy time (proning), is something being done in people with lung damage from COVID.

Hope that helps, and hopefully he gets evaluated soon. But honestly, he may be someone that a month ago they would've admitted, but they may send him. It's just a bad situation and there's no good solution.

He finally got into a room in the ER now. He said his chest x-rays checked out fine and they did some other tests that came back fine, but he couldn't tell me what tests. He did say the nurse had him get up to walk around and he had to catch himself on the bed because he felt like he was going to pass out/collapse and he's never felt like that before. They just had a shift change so he's waiting on the new dr to come in to see what's next. They told him there is nothing they can give him for COVID, just that they could treat pneumonia if he were to get it. What about the antibody therapy and/or other meds? Am I mistaken in that there are some treatments they could try? He's at Scott & White and we haven't exactly had the best experiences with them with other family members. I have very little confidence in that hospital.

Well, now they are sending him home with cough medicine and to keep monitoring his oxygen levels and come back if needed. Said he didn't meet the criteria to be hospitalized. Not sure how I feel about that. He doesn't sound good when I talk to him and I feel with his age and his asthma he needs some meds or something.

Most of the treatments for COVID that are advertised honestly aren't that great. The mainstay of therapy is still supportive care till the immune system clears the virus and heals. The only treatment that shows a clear benefit are steroids, specifically one called dexamethasone. May or may not be useful for your father, depending on his condition.

Things like remdesivir, regeneron, convalescent plasmsa, etc., have debateable benefit. At least in people with mild disease that don't need supplemental oxygen. A daily baby aspirin might be of some benefit. Dr. Rev's supplement cocktail probably won't hurt anything either.

If hospitals weren't overwhelmed right now I think they may be more inclined to hold onto patients and try some of these therapies in the less sick patients. I just don't think they can spare the room, time, and resources.

The barometer for what needs hospitalization may have changed over the past few months. But the silver lining here is that they saw him and didn't think he was sick enough to need admission. Just a couple more days and he should hopefully be over the hump.

He says he's still feels about the same, although his oxygen levels did come back up. He's supposed to talk to the dr on the phone later today. His sense of smell and taste are coming back, so hopefully he's on the back end of this thing and will be completely over it soon. Still have some concerns though. My moms cousin went to the ER the same day my dad did and they sent him home as well. However, he was brought back to the ER this morning and is going downhill rapidly.
