This could be purely anecdotal, but I have now heard of two people who received their first dose of mRNA without knowing they were presymptomatic. Both had symptom onset within a day or two after getting their shots, and went on to have severe disease, one (healthy, 30s) has been on a ventilator for weeks, the other (healthy, 60s) did not survive.
Is this a known problem, or just a matter of consequence? It makes sense to me that vaccinating a presymptomatic person could be problematic, as the immune system is dealing with peak viral load and then could be overwhelmed by the vaccine induced response. I'm not medical so I have no idea, just wondering if this is a documented issue.
