Can see how this would happen. The Pfizer vaccines ship in multi-dose vials. We were getting up to six doses per vial at the vaccine clinic.

There were a lot of checks at the mass vaccination areas, with people drawing the vaccines being double-checked by someone else who is keeping track of the total doses, and the person giving the shots had nothing to do with the drawing up. It takes more people but it is safer and faster.

We will probably hear about more of these overdoses until they can produce single-dose vials as it will be more likely that one person will be drawing up and administering each shot.

Easy to make single dose vials. Problem is that we want lots of doses out quickly. Multidose vials reduce the number of vials, make shipping easier, etc. We won't get single dose vials until we stop the massive rollout of vaccines.
