and has bloodwork done at 8 am after drinking until 1 am, wouldn't their bloodwork show something? Asking for a friend whose doctor never seems to see anything

They don't check BAC on normal blood work. Imagine some indicators like ALT, AST might be slightly elevated but probably not enough to set off real warning bells. Usually takes more than months of abuse to really cause bad things to happen in your body. Liver will really put up with a lot.

You can kill about 80% of your liver before you start showing sequelae of alcoholic liver disease.

ALT, AST, ALK Phos which is what gets tested on a cmp/lft (common lab work) usually will be normal because those are acute phase reactants. So an acute process will elevate those enzymes but chronic processes will not.

Lab findings like albumin and inr are measures of protein that the liver produces. When those levels start becoming abnormal then it's already too late and you have cirrhosis which is irreparable harm to your liver.
