looking at the video, that looks like some sort of steering casualty to me. Looking at the wake, it looks like they have a right standard (15 degrees) rudder on at about 15 knots (typical approach for an unrep). You don't ever turn that hard towards the oiler while coming alongside. You set your course well in advance at least 500 yards astern and then slowly move up making minor 2-3 degree adjustments as you slowly catch up to the oiler.

Having all the experience of docking various small boats on lakes over the past 30 years, this just looks terrifying to me. Surprised more collisions/accidents don't happen, really.

Agree with the above that it looks like one engine must have accelerated/not responded to commands on the ship that slammed into the re-supply one. No idea though, and no one would want me to run anything like this type of operation.

I would agree with this. If it was a steering error, they would have at least attempted to make a correction, which they didn't.

Yep, and they weren't close enough for Venturi to take over after a minor helm dyslexia moment. Even with major helm dyslexia, OOD, CO, helm safety, anyone, would be able to get to the helm in 5 seconds and fix it. That didn't appear to happen.
