Earlier today I was bending over picking up a trash bag (not heavy) and this crazy sensation came over my lower back. Hurts like hell ever since. Any tips on dealing with this and preventing freak stuff like this in the future? I can barely walk.

This is 42.

Get on a heating pad tonight. Take a long warm bath. Take NSAIDS. Long term get stronger but sometimes these things happen.

It sounds like you have a back spasm. Pain can be severe and very debilitating. I have had this a few times in my life. Usually after something very simple like bending over, sneezing, getting out of a chair, etc. No body understands your pain unless they have had them.

Rest, heating pads, pain relievers, icy hot pain patches and warm baths are most of the things I do. If you have someone to help, low level vibrating massage is useful in my opinion. These can take weeks to fully recover from. Should start to see some improvement in 4-5 days.

One thing I wish is that I would have gone to a PT after my first one to learn back and core exercises.

Some pain docs and PTs I know say that seeing a PT ASAP can be helpful even in the stage you are in. I have not done this so don't have personal experience.

Some warning signs indicating you need more urgent attn from an MD would be fever, weakness in your legs or bowel/bladder incontinence.

Good luck. These spasms are absolutely terrible.

I'm 54. Same. I did four miles on the treadmil on Friday l and wasn't able to much yesterday due to pain. Back is killing me. May be sciatica.

I'm also having spasms in my buttocks and hammies on my left side. Plan is ibuprofen, stretching and heat. Probably should do ice but it's too cold. Lol.

GF had some left over muscle relaxers and i took one of those last night.

Got a short massage from the GF and that was awesome. Plus it lead to other things that took my mind off the back pain.

Excellent book on back pain:

Back Mechanic - Dr. Stuart McGill

One of the best things I have found to help provide relieve is spine decompressions along with back bends. Hang from a pull-up bar with your feet / toes touching the ground and relax your upper body allowing your spine to decompress (a minute or two). Follow that up with back bends off the floor (3 or 4 times for 10 to 15 seconds). Lay on the floor on your stomach and push your torso up with your hands, arching your lower back. If that's too painful initially, simply lay on the floor and start to prop yourself on your elbows, as far as you can. Lay there for a good 10 minutes or more until things start to relax. Do this several times a day, as often as you can, until things start to calm down. Avoid any forward bending, anything that rounds your lower back. That's just going to make things worse.

I know this sounds like a waste of time, but I found just walking like two miles at a slightly elevated pace followed by a heating pad does absolute wonders.

(That and start lifting heavy weights over the long term).

This is absolutely true. One of the things that McGill recommends as part of recovery is walking, ideally 3 times a day for about 20-30 mins each.

50 year old active guy here who lifts and plays a lot of golf. Here's what I've found works best when I tweak something.

First 24 hours is minimizing initial inflammation. NSAIDS and ice/cold packs. You are trying to keep the swelling from that initial injury minimized, because that swelling is what will compress nerves and cause all sorts of fun. Then start walking day 2. Good brisk walk multiple times per day but then ice again afterward. You want to get your body working ASAP but still want to manage inflammation from that. I have found that heat just aggravates things.

After a few days, start getting some gentle stretching going. I favor some yoga moves like downward dog, childs pose, and cobra. Very gentle at first. Ease into it. Not a lot of twisting. You don't want to get carried away stretching an injury trying to heal, but maintaining flexibility is important
