Has anyone tried repairing this non surgically with PRP, BPC-157, or anything else combined with PT? At this point if I can get back to 80% and forgo surgery I may consider it

Why? The surgery is easy. Just get it done.

8-12 month recovery is not what I call easy

Find a new doc/PT. It's not that long to functional recovery. Besides, your method will take at least half that and still may meed surgery when it fails. Arthroscopic surgery is the way to go. Unless your a pitcher.

I am a very active person and would like to do BJJ again. Doc said 4-6 months and then I can start getting into easy lifts, a year or so for BJJ

Had slap tears, plus minor rotator cuff issues, and other long term shoulder issues in both shoulders at the same time.

Concentrated on the right first, I'm a righty. Went the PT route, also had steroid shot, and 1 round of PRP injection. Choose not to get surgery, so that I could continue working (outdoor based job) and really didn't have the time to be out of commission. PT helped, especially once work let me off from the more strenuous work. Started bowling within a year of the diagnosis, was pretty much good the last couple years until this Jan when I aggravated it (now dealing with frozen shoulder).

When I started concentrating on the left shoulder doing PT, I wasn't truly happy with the PT doc. Because during the first go around I was already doing exercises that worked both sides (band work and other exercises), and the 2nd doc wanted me doing light weights (which I demonstrated I could do either heavier or a carp ton of lw reps).

Go with quality PT first and then let you and the ortho doc determine if surgery is truly needed. I'm sure I'll finally get the surgery sometime in the future and I'll be out of commission for awhile due to putting it off.

Had a bad fall in a hotel shower. Slipped on a wat floor, put an arm out to catch myself and caught the edge of the tub and jacked my shoulder. For sure tore something. Getting a t-shirt on was excruciating. Never had an MRI, but doc via movements and range of motion and pain confirmed I tore something.

Length of recovery time was my concern as well, so I got to it. I lifted around it at the gym. Seated-row movements never hurt too much, so did a lot of similar exercises, pressing movements on fixed machines before I could manage free weights, and lots of lower body stuff. Lots of machines helped me recover to the point that I felt safe without them.

It took a few months, but I got stronger ang stronger and the only impact I have no is a slightly limited range of motion on the arm compared to the other.
