As an ER doc, I would second going to the ER. They can probably help you get a little better relief.
As for why a doctor wouldn't prescribe? Sometimes its as simple as they can't.
When I renew my DEA license every so often, I have to click the levels of controlled substances that I am allowed to prescribe. II for example allows you to write hydrocodone, oxycodone. III for Tylenol 3 and tramadol.
There are many doctors out there they simply never click a II level prescribing authority. They choose to not prescribe "the stronger stuff". What you prescribe is still very much up to the prerogative and the decision of the prescriber. If you don't click to prescribe II, you can't legally.
Now, personally, if I were a surgeon and doing very painful things to people, I would "choose" to write for the stronger stuff myself, and I do, as an ER doctor, but it's still prescriber choice and preference.
