My dad died unexpectedly last week, and our few family members were together (masked) periodically Friday-Sunday. Sunday afternoon, my brother developed a fever, and today received a positive test result.

My wife and I developed dry coughs late yesterday/this morning. Though we'll get tested, it's probably safe to assume that we have it.

I have Dr. Rev's protocol (zinc, quercetin, etc.), but I seem to recall a suggestion of things to do immediately upon onset (larger dose Vit D, etc.). Just can't find the thread that I saw it in. Can anyone point me to it?

First I start with Zinc up to 50mg twice per day, Quercetin 1g up to 2 twice per day or Green Tea 500mg twice per day to act as a zinc ionophore. The purpose of this is to hopefully inhibit the enzyme replicase and thus slow viral replication.

Next, I use Vitamin D up to 50,000IU at the time of diagnosis. In a recent hospital trial they gave 22,000IU on admission then 11,000IU on day 3 and day 6 then weekly. In that trial there were 50 in the treatment group and only 1 went to the ICU in the nontreatment group there was 26 and 13 went to ICU and 2 died. Since vitamin D toxicity is very very rare I have everyone take high dose Vit D. In addition another retrospective study of over 190,000 patients of Vit D showed over a 50% reduction in
contracting the SARS-COV2 virus.

Next I prescribe famotidine 20mg twice per day. In a retrospective analysis posted on medRxiv of roughly I1,300 hospitalized COVID-19 patients, they found that individuals taking proton-pump inhibitors had a two- to three-fold higher risk of death compared with hospitalized patients not taking those drugs. Another study published in late August in the American Journal of Gastroenterology by doctors in the US that also found people taking a proton-pump inhibitor twice a day for acid reflux had higher odds of testing positive for SARS-CoV-2 compared with individuals taking that type of drug once a day or individuals who took a histamine-2 receptor blocker such as Pepcid AC. Another retrospective analysis published online in Gastroenterology in May found that in a cohort of 1,620 hospitalized patients, proton-pump inhibitors had no relationship to the patients' outcomes. Meanwhile, famotidine, which blocks the histamine-2 receptor on cells, correlated with a reduced risk of patients being intubated or dying from COVID-19.

Next I have them take melatonin at night. Analysis of patient data from Cleveland Clinic's COVID-19 registry also revealed that melatonin usage was associated with a nearly 30 percent reduced likelihood of testing positive for SARS-CoV-2. Notably, the reduced likelihood of testing positive for the virus increased from 30 to 52 percent for African Americans.
Next I use a baby aspirin as long as they have no contraindication. We have frequently discussed that Covid-19 is likely an endothelial disease that causes clots to form. So preventing clots is paramount. A recent retrospective paper found that people taking aspirin were 44 percent less likely to be put on a ventilator and 43 percent less likely to be admitted to the intensive care unit (ICU). The researchers also reported that those who took aspirin had a 47 percent decrease in risk of dying from COVID-19 when compared with those studied who were not taking a daily dose of aspirin. "Our analysis suggests that aspirin use may have beneficial effects in patients with COVID-19," the study authors wrote in their study.

Next I have them take either NAC 600mg BID or Coenzyme Q 10 200mg per day to decrease the oxidative stress in their blood vessels. I personally think NAC is likely better as it also has some anticoagulant and platelet-inhibiting properties. Usually CoQ10 is easier to find but NAC is readily available on amazon prime.

Next I use vitamin C 1000mg 1-2 tabs twice per day. The data on Vitamin C however is sketchy at best. It was studied in IV form only and steroids were added which could have potentially contributed to some of the benefits in the study. However, it is an antioxidant and free radical scavenger that has anti-inflammatory properties, influences cellular immunity and vascular integrity, and serves as a cofactor in the generation of endogenous catecholamines. Because humans may require more vitamin C in states of oxidative stress, vitamin C supplementation has been evaluated in numerous disease states, including serious infections and sepsis. So for these reasons it is reasonable to take.

Finally, I use steroids in patients with risk factors. I usually use oral prednisone or inhaled budesonide. I have them all get a pulse oximeter and watch for readings <92%. If that occurs I for sure add inhaled budesonide and albuterol. If <90% I tell them they will have to go to the hospital.

While we don't have proven treatments yet, outside of steroids and aspirin these things have very little risks and all have some potential benefits. I inform the patients that they are not FDA approved proven treatments but I have personally seen good results with them. There is also lots of data on secondary bacterial infections with Covid-19. So I watch for this and consider doxycycline if patients look bad or call back and are not doing good. Hopefully this helps some.

The Vitamin D is the key. My sister, brother in law, and nephew all got it (brother in law's dad was confirmed close contact at Thanksgiving, he ended up in the hospital with it).

I had shared Doc Rev's treatment regimen with them. They did the 50K vitamin D megadose, zinc, and NAC 500mg twice daily. They all kicked it by day two.

Brother in law's dad was in hospital for three days - they gave him remdesivir, walked out of hospital under his own power after going in on a stretcher.

Is taking a 50k dose the same as 10 5k's? I understand the math checks out but wondering if it really is that simple. I have struggled with vitamin d for years and the thought of taking 10 at once is a bit worrisome.

I'm almost certain that Rev has said it's okay to split the dose.
