I'm not clear on whether the Lady of the Lake from whom King Arthur received Excalibur was entirely made up by Sir Thomas Malory as part of "Le Morte d'Arthur" or if there was an earlier version, but the Welsh Mabinogion has the "Sword in the Stone" tale regarding Arthur so that could be Mallory's inspiration.
