Bariatric Ambulances. Some of the private companies have had them for a while. The suspensions are heftier, they have ramps to get a stretcher up and in - often with a winch, and then the 18 wheeler equivalent of the stretcher itself. If having the stretcher hauling your ass use a pair of 24V Dewalt Lion batteries to run the gears raising and lowering don't encourage you to swap a taco for a salad now and then, perhaps having your MRI's and X-rays at SeaWorld will.

As long as accommodations are made, they'll never stop
