Three things:
1. I didn't even think about Ted being dead. I just thought Skyler was feeling depressed about living with a gangster that scares the hell out of her and because Ted likely won't walk again. But I guess it's possible Walt had him killed somehow. If he did, it certainly wasn't with the ricin, which we saw Walt hide in the outlet behind his nightstand.
Did Walt actually say that he'd "take care" of Ted? I don't remember that.
2. You could see the wheels turning for Hank when the boss was talking about Gus being right under his nose. He has to be very close to putting it all together--the chemistry, W.W. in Gale's notebook, Walt's "gambling winnings," Walt's bizarre behavior and car wreck in front of the laundry, and the fact that Walt refused to join the family on lockdown when Gus was bombed and the lab was burning up.
3. What caused Mike to have a change of heart while he had Lydia at gunpoint?
At the diner, Mike told Lydia those 11 guys were solid and Gus had paid them well to be quiet. Standing there with his gun on Lydia, did he realize that with the feds seizing all of the offshore "hush money" accounts, he had to either get the cash flowing again or kill everyone?
